Care plans: Glossary of terms

A quick reference for the words you'll come across when working with Care Plans in Lookout.

The Basics

Care plan

A shared agreement outlining the support a Member will receive and how it will be delivered. It gives Helpers and Members clarity, and forms part of the ongoing health record between the Member and your organisation. (You may also see it called a help plan or support plan.)

Care plan overview

The at-a-glance view of a Member's plan, bringing together accessible clinical information in six sections - Summary, Directives, Clinical Summary, Conditions, Medications, and Allergies - plus any Top Active Risks.

Category

A structured section of a care plan. For example, "Who I am," "Goals," or "How to support me." Categories organise the plan and can hold multiple items. The number shown next to a category tells you how many items sit within it.

Item

An individual entry within a category. Categories can include preset items and custom items you add yourself.

Template

A reusable starting point that defines a care plan's structure - its categories, items, and the prompts that guide staff. Templates standardise care planning across your organisation.

Mandatory category

A category that must be completed before a care plan can be saved. In the standard setup, these are "Who I am," "Goals," and "How to support me." Other categories are optional.


Overview sections

Top active risks

The risks identified for a Member, aligned to the Strengthened Aged Care standards. These apply to HCP-transitioned, Support at Home, and CHSP funding scheme Members.

Summary

A high-level overview of the Member and their care needs.

Directives

Advanced health directives or key directions relevant to the Member's care delivery.

Clinical summary

Recent clinical context - relevant assessment findings and care considerations.

Conditions

Health conditions relevant to supporting and delivering care.

Medications

Medication information relevant to care planning.

Allergies

Allergy information relevant to safe care delivery.


Personalisation & Risk

Goals

What the Member would like to achieve, captured on the plan.

Service preferences

How the Member likes their support and services delivered.

Strengths

The Member's individual strengths, reflected in the plan.

Risk management

The risk category, level, reasoning, and mitigations tracked directly on the care plan.


Managing Care Plans

Track time

Logging care management minutes spent creating or updating a care plan, recorded in 15-minute increments.

Export to PDF

Downloading or printing a care plan as a PDF to share with the Member and their loved ones.

View History

The audit trail of changes made to a care plan - showing who made each change, what changed, and when.


Lookout Assist

Lookout Assist

Lookout's AI-powered feature that helps generate and update care plans by drawing on uploaded documents and information, producing a draft for your team to review, edit, and approve.

Draft care plan

A plan generated by Lookout Assist that is not yet active. It must be reviewed, edited if needed, and approved before it becomes the Member's active care plan.

View sources

A Lookout Assist feature showing which documents and information a draft was based on, and flagging any issues to review before publishing.

Care plan event

New information that prompts Lookout Assist to propose an update - such as a submitted form response, an incident action sent to the care plan, or an observation investigation sent to the care plan.

Sparkle Icon

The marker Lookout Assist adds next to a category to show it has been updated in a proposed change.
