A guide on completing a Falls Risk Assessment Tool (FRAT) with the help of Lookouts Focal Assessment add-on.

The Falls Risk Assessment Tool (FRAT) is an instrument to identify possible risk factors contributing to falling and document an action plan on specific areas surrounding the individual's fall risk.

๐Ÿ’ฌ The Focal Assessment add-on is required to complete a Falls Risk Assessment Tool (FRAT) assessment.

Completing a new assessment

The Falls Risk Assessment Tool (FRAT) has four steps:

  1. Fall Risk Status

  2. Risk Factor Checklist

  3. Action Plan

  4. Transfer to Help Plan

Step 1 - Fall Risk Status

To complete a new FRAT assessment, navigate to the Lookout tab on a Membership and click 'New Assessment'.

Select the Falls Risk Assessment Tool (FRAT) assessment.

Select the date for the assessment and proceed to answer the questions provided.

As you complete the questions, your assessment score will be displayed.

Ensure that you click 'Save' once the assessment questions have been completed.

During the assessment process, Lookout scans a member's health timeline for any fall events and presents this history.

Step 2 - Risk Factor Checklist

Click 'Edit' within 'Part 2: Risk Factor Checklist' and details any risk factors.

Step 3 - Action Plan

The Action Plan will populate based on the Risk Factor Checklist in Step 2. Click 'Edit' to document strategies for the identified risk factors and how they will be managed.

Step 4 - Transfer to Help Plan

The completed assessment can be transferred to the client's Help Plan with additional notes and a review date.

The ticket can be closed once all assessment steps have been completed.
